AWANA is Skiff's Awesome Midweek Kid's Ministry & is all about reaching kids with the Gospel of Jesus Christ & teaching them to follow Jesus as they learn to hide God’s Word in their hearts.

Your child will enjoy lessons, songs, Bible memory, games & more!

Cost = $15. Each additional child = $5. Max $30/Family.

Scholarships Available, please ask if needed.

Consent and Medical Authorization

I, the undersigned, being the parent or legal guardian of the child named on this online form, do hereby consent to the participation of my child in AWANA at Skiff Lake Bible Church.

Medical Authorization: I understand that I will be notified in the case of a medical emergency. However, in the event that I cannot be reached, I authorize the calling of a doctor and the providing of necessary medical services in the event that one or more of my children are injured or become ill. I authorize a representative of Skiff Lake Bible Church to make emergency medical care decisions on behalf of my child/ren, if required by law or a health care provider: I authorize these persons to act in my place to consent to all necessary and appropriate x-ray examinations, anesthetic, medical or surgical diagnosis or treatment, and hospital care. I understand that Skiff Lake Bible Church will not be responsible for medical expenses incurred solely on the basis of this authorization.

I also understand that the adult supervisors reserve the right to restrict my child from any activity that they do not feel is within the physical capabilities of my child/ren.

Photo / Media Acknowledgment

I understand that photos and/or videos may be taken of my child during AWANA for church use (slides, website, social media, etc.). I agree to expressly release Skiff Lake Bible Church (including her directors, membership and staff) from any and all claims which I may have for invasion of privacy, right of publicity, defamation, copyright infringement or any other causes of action arising out of the use, adaptation, reproduction, distribution, broadcast or exhibition of such photographs, videos, voice recordings and the like.

**Skiff Lake Bible Church reserves the right to use any photograph or video taken at any service on church grounds, or event sponsored by Skiff Lake Bible Church, without the expressed written permission of those included within the media form.
